The LPN assists with patient assessments, provides direct care, and educates patients and families to promote health, safety, and independence.

Position Summary
Provides personal care and supportive services to patients in their homes under the supervision of a Registered Nurse (RN) or licensed clinician. Assists with activities of daily living (ADLs), monitors basic health needs, and helps promote comfort, safety, and independence for patients receiving home health care.
