
​Title: Registered Nurse (RN)
Employment Type: Full-time / Part-time / PRN
Position Summary
The Registered Nurse (RN) provides skilled nursing care to patients in their homes under the direction of a physician and in accordance with the agency’s policies and state/federal regulations. The RN is responsible for assessing patient needs, developing and implementing individualized care plans, coordinating with the healthcare team, and promoting patient independence and well-being.
Key Responsibilities
•Conduct initial and ongoing assessments of patients’ physical, emotional, and social needs.
•Develop, implement, and update individualized care plans in collaboration with physicians and interdisciplinary team members.
•Provide skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
•Educate patients and families on disease management, treatment plans, and home safety.
•Monitor patient progress, document observations, and report changes in condition to the physician and agency leadership.
•Supervise and provide guidance to Licensed Practical Nurses (LPNs), Home Health Aides, and other care staff.
•Ensure compliance with all agency policies, procedures, and regulatory requirements.
•Participate in case conferences, staff meetings, and continuing education activities.
•Promote patient independence and self-care while ensuring dignity and respect.
Qualifications
•Current, active Registered Nurse (RN) license in Maryland.
•Home health experience preferred, but not required..
•Knowledge of home health regulations, OASIS documentation, and Medicare/Medicaid guidelines (preferred).
•Strong critical thinking, assessment, and communication skills.
•Ability to work independently and manage time effectively.
•Valid driver’s license, auto insurance, and reliable transportation.
Physical Requirements
•Ability to travel to patients’ homes and provide hands-on care.
•Must be able to lift, position, and transfer patients as needed.
•Frequent standing, walking, bending, and reaching.
