Title: Home Health Aide (HHA)

Position Summary

The Home Health Aide (HHA) provides personal care and supportive services to patients in their homes under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN). The HHA assists with daily living activities, helps maintain a safe home environment, and offers companionship and emotional support while promoting patient comfort and independence.

 

Key Responsibilities

•Assist patients with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and feeding.

.•Provide mobility assistance, including transfers, ambulation, and positioning.

•Perform simple health-related tasks such as taking temperature, pulse, and respiration (if permitted by state regulations).

.•Observe, document, and report changes in patient condition to the supervising nurse.

•Assist with basic household tasks, such as light cleaning, laundry, and meal preparation related to patient care.

•Provide companionship, emotional support, and encouragement.

•Follow the patient’s care plan as directed by the supervising nurse.

•Practice infection control and safety measures at all times.

•Protect patient privacy and uphold dignity and respect.

 

Qualifications

•Completion of a state-approved Home Health Aide (HHA) training program recognized by the state.

•Active listing on the state registry of nurse aides.

•Previous experience in home health, hospice, nursing home, or hospital preferred, but not required.

•Strong communication and interpersonal skills.

•Compassionate, dependable, and patient-centered.

•Ability to follow instructions and work under supervision.

•Valid driver’s license, auto insurance, and reliable transportation.

 

Physical Requirements

•Ability to lift, move, and transfer patients.

•Frequent standing, walking, bending, and reaching.

•Ability to travel to patients’ homes.

Title: Certified Medication Technician (CMT)

Employment Type: Full-time / Part-time / PRN

 

Position Summary

The Certified Medication Technician (CMT) provides safe and accurate administration of medications and treatments to patients in their homes under the supervision of a licensed nurse (RN or LPN). The CMT also assists with basic patient care, observes and reports patient responses to medications, and supports overall health and well-being in accordance with the care plan.

 

Key Responsibilities

•Administer oral, topical, eye, ear, and other approved medications as delegated by an RN or LPN.

•Ensure proper documentation of all medications administered, including dosage, time, and route.

•Monitor patients for side effects, reactions, or changes in condition and report to supervising nurse.

•Provide basic health-related tasks such as taking vital signs, weight, and blood sugar checks (if permitted by state regulations).

•Assist with activities of daily living (ADLs) as needed, including bathing, grooming, dressing, and mobility.

•Maintain medication storage, safety, and infection control standards.

•Document patient care activities accurately and in a timely manner.

•Provide emotional support and encourage patient independence.

•Follow agency policies, state regulations, and scope of practice guidelines.

 

Qualifications

•Current Certified Medication Technician (CMT) certification in the state of practice.

•Active listing on the state nurse aide or medication technician registry (if required)

.•Prior experience in home health, long-term care, or healthcare setting preferred.

•Strong attention to detail and commitment to medication safety.

•Compassionate, dependable, and able to work under supervision.

•Valid driver’s license, auto insurance, and reliable transportation.
