Title: Licensed Practical Nurse (LPN)

Employment Type: Full-time / Part-time / PRN

 

Position Summary

The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN) and in accordance with the physician’s orders, agency policies, and state/federal regulations. The LPN assists with patient assessments, provides direct care, and educates patients and families to promote health, safety, and independence.

 

Key Responsibilities

  • Provide direct patient care, including wound care, catheter care, medication administration, and monitoring of chronic conditions.

  • Assist the RN with patient assessments and development of individualized care plans.

  • Observe and document patient status, reporting significant changes to the RN or physician.

  • Administer prescribed treatments and therapies as directed.

  • Educate patients and families on disease management, medications, and home safety.

  • Support patients with activities of daily living (ADLs) when needed.

  • Supervise and guide Certified Nursing Assistants (CNAs) and Home Health Aides in carrying out care plans.

  • Maintain accurate, timely, and complete clinical documentation.

  • Follow infection control, safety, and quality standards.

  • Participate in case conferences, staff meetings, and continuing education.

Qualifications

  • Current, active Licensed Practical Nurse (LPN) license Maryland.

  • Home health experience preferred, but not required..

  • Knowledge of home health regulations, OASIS documentation, and Medicare/Medicaid guidelines (preferred).

  • Strong clinical, communication, and organizational skills.

  • Compassionate, dependable, and able to work independently.

  • Valid driver’s license, auto insurance, and reliable transportation.

 

 

Physical Requirements

  • Ability to travel to patients’ homes.

  • Must be able to lift, move, and transfer patients as needed.

  • Frequent standing, walking, bending, and reaching.
