
Title: Licensed Practical Nurse (LPN)

Position Summary
The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN) and in accordance with the physician’s orders, agency policies, and state/federal regulations. The LPN assists with patient assessments, provides direct care, and educates patients and families to promote health, safety, and independence.
Key Responsibilities

Provide direct patient care, including wound care, catheter care, medication administration, and monitoring of chronic conditions.

Assist the RN with patient assessments and development of individualized care plans.

Observe and document patient status, reporting significant changes to the RN or physician.

Administer prescribed treatments and therapies as directed.

Educate patients and families on disease management, medications, and home safety.

Support patients with activities of daily living (ADLs) when needed.

Supervise and guide Certified Nursing Assistants (CNAs) and Home Health Aides in carrying out care plans.

Maintain accurate, timely, and complete clinical documentation.

Follow infection control, safety, and quality standards.

Participate in case conferences, staff meetings, and continuing education.

Qualifications
-
Current, active Licensed Practical Nurse (LPN) license Maryland.

Home health experience preferred, but not required..
-
Knowledge of home health regulations, OASIS documentation, and Medicare/Medicaid guidelines (preferred).

Strong clinical, communication, and organizational skills.

Compassionate, dependable, and able to work independently.

Valid driver’s license, auto insurance, and reliable transportation.
Physical Requirements

Ability to travel to patients’ homes.

Must be able to lift, move, and transfer patients as needed.

Frequent standing, walking, bending, and reaching.
